สรุปผลการประชุมการจัดทำกรอบบัญชียาร่วม เขตสุขภาพที่ 7

  1. กรอบบัญชียาร่วมเขต X Z R ที่ได้จากมติที่ประชุม CPO ในวันที่ 16 ก.ค. 68 ขอให้ใช้เป็นบัญชียาส่งต่อระดับเขตสุขภาพที่ 7
  2. หมายเหตุ รายการยา "X"เป็นรายการยาที่โรงพยาบาลต้องมีใยบัญชียาหน่วยบริการตน (minimum list)
  3. หมายเหตุ รายการยา "Z"เป็นรายการยาที่ผู้ป่วยเริ่มรักษาที่รพ.แม่ข่าย แล้วส่งต่อผู้ป่วยกลับไปดูแลต่อยัง รพ.ลูกข่าย โดยจะส่งมอบยาไปพร้อมกับผู้ป่วย 14-30 วัน เป็นยากลุ่มโรคเรื้อรัง หรือตัองใช้เวลาในการรักษามากกว่า 3-6 เดือน
  4. หมายเหตุ รายการยา "R" เป็นรายการยาที่โรงพยาบาลแม่ข่าย ส่งต่อไปยังโรงพยาบาลใกล้บ้าน โดยจะส่งมอบยาไปพร้อมกับผู้ป่วยจนครบการรักษา ได้แก่ ยาต้านจุลชีพ และที่มีมูลค่าสูง
  5. นัดประชุมครั้งถัดไป: วันที่ 21-23 ก.ค. 68 เวลา 13.30 น. เป็นต้นไป โดยกลุ่มเป้าหมายประกอบด้วย ผอ.รพ. และหัวหน้ากลุ่มงานเภสัชกรรม ของ รพ. A/S/M1/M2 เพื่อชี้แจงบัญชียา/Drug Warehouse และขอมติการบริหารจัดการ
